AT & F approach to home care is "care coordination". This new concept differs from traditional one since it consists of a disciplinary team of health and human
services professionals, all employed by the same company, dedicated in helping patient remain and thrive at "home"

The difference of AT & F with other home care providers:
* License of Illinois Department of Public Health (IDPH), Bonded and Insured.
* Manage and owned by individuals who have ten (10) years clinical experience in Home Care Service.
* Applicants are subjected to background check, including criminal, references, motor vehicle and employment verification.
* Matches the care giver's behavior with the patient.
* Care giver is required a four (4) hours training before they are allowed to start the work. The training emphasizes the patient assessment and the care plan.
* Conduct the patient assessment and prepare a care plan before acceptance of patient.
* Care giver's training is done quarterly.
* 24/7 on-call is available. All Case coordinators are ready to answer questions and assists the care giver in time of need. Communication is supported with
the care giver's cell phone and internet.
* Agency case coordinators conduct regular on-site visit as part of our commitment for excellent service.
* Care home service is very affordable. We do not require advance payment.

Patients are informed of their rights to privacy of medical information. AT & F Home Care, Inc. and all its contractual providers are in full
compliance with HIPAA requirements.

It is the policy of AT & F Home Care, Inc. not to discriminate any patient on the basis of race, color, national origin, language, sex, or
religion in providing home care services. Our open admission policy will not deny anybody with contagious disease, including but not
limited to AIDS, MRSA, and Hepatitis B.
We will not decline admission or limit service due to severity of medical condition; membership in or affillation with a religious or fraternal
group; location of services or facilities lacking handicap access. We will provide the necessary assistance to patients with sensory or
speech impairments.
